The NHS Teen LifeCheck
A quick quiz-style online questionnaire for young people aged between 12 and 15.

It has been designed to help young people to take greater control of and responsibility for their health and well-being by raising their awareness of risk and making them aware of local and national support services available to them.

The questionnaire takes about 10 minutes to do and gives young people a personalised profile at the end.
Teen LifeCheck has been developed by the Department of Health.
